My Story Interviews
The greatest legacy we can leave behind is the story of a well lived life and inspired relationships. A human story is a collection of values, ideas, moments, and relationships. A My Story Interview captures the Ideas, dreams, morals, and values used to guide life’s most meaningful relationships, principles, values, dreams and wisdom in order to share them as part of an enduring legacy. A legacy consultant will conduct and record an interview discussing the life, relationships, history, morals, values, meaningful moments in life and a personal message to inspire other to strive for a well lived life. personalized messages can be meant for a group and/or individualized.
Advanced Care Planning
Advanced care planning is more than just a set of documents or code status. It is about intentionally setting forth the values and wishes of what care is desired for the various stages of life and sharing them with those who will be part of the decision-making process. Our Draw Your Line (DYL) care map document helps to guide the decision-making process to ensure the care received is the care wanted and aligns with the values and wishes of the individual. This service includes an initial in-home educational session to help navigate the complexities of modern healthcare, along with identifying what health means to the individual and their values and wishes for care. Once health, priorities and values are identified there is a second in-home planning session to share them with the decision-making team and review all documents. Included with the advanced care planning are documents to establish other advanced directives such as Durable Power of Attorney for Healthcare (DPOA-H), Durable Power of Attorney Financial (DPOA-F), and Funeral Representative Designation (FReD) with copies provided for each decision maker.

The Comprehensive Legacy combines Advanced Care Planning, a My Story Interview, Financial Legacy organization, Aging in Place Consult, and Funeral Planning Assistance to ensure the preservation of an enduring legacy and inspire others towards a well lived life.
Financial Legacy Organization
Organizing finances if a vital part of legacy planning and alleviating stress during life transitions. Ensuring bills are paid, responsibilities are met during life’s transitions is vital in preserving a legacy. A legacy consultant will work to help catalogue and organize bills, liabilities, accounts and other assets into a legacy folder listing banks and institutions where the client has accounts. A legacy folder helps families focus on health and relationships during transitions rather than piecing together a financial puzzle. A legacy consultant can also help prepare for life’s final transition by helping catalogue and organize assets and document a plan for how they are distributed.
Aging In Place Consult
A home is more than a living space. For many it is the place where relationships blossomed, families grew, and love flourished. More than 80% of Americans wish to age in place and die at home, less than 20% do so. With careful planning and early investment, it is possible to stay in your homes as long as possible. By designing or redesigning living space to meet changing needs throughout life’s journey it is possible to age in place and die at home. Each individual has unique needs as they navigate the various stages of life. An aging in place consult will help identify necessary modifications before they are needed. Early modifications can help reduce stress and shorten hospitalizations or rehab stays and preserve a financial legacy. Common modifications are roll-in showers, ramps, grab bars, and laundry and all other needs on the main floor.
Bucket List Assistance
A legacy is more than what is left behind, it is about the moments and memories made during life. Life’s journey often unpredictable and takes unanticipated detours. It is easy to put off dreams and goals until a decline in health make them seemingly impossible. A legacy consultant can help plan that dream vacation, gathering, or activity and accompany as a personal companion and caregiver. Our legacy consultants include licensed medical professionals who are able to facilitate events otherwise impossible due to a decline in health or mobility.
Funeral Planning Assistance
Leaving a final farewell to the people who made life meaningful is the cherry on top of life’s sundae. Planning your final celebration significantly reduces stress, reminds friends and family they are loved and creates the space for healthy mourning and a celebration of a well lived life. A legacy consultant will help plan a posthumous funeral and or a living funeral. Preplanning a funeral takes the stress of planning it off of loved ones, facilitates togetherness, and ensures no one will see that photo you hate. Planning ahead means you are able to leave behind the meaningful message you desire and inspire others towards a well lived life. A living funeral is a one or more celebrations of life with those who helped give life meaning and sharing all the “I wish I would have said…………”s while we are still able to say them.
Relationship Reconciliation
Relationships are hard and miscommunication, misunderstandings, and differing perspectives often lead to estrangement and separation. At Legacy Planning we believe relationships are the purpose of human existence and the central element of every legacy. Fostering healthy relationships in life, and leaving inspired relationships when we are gone, is the core value on which legacy planning was created. The things left unsaid are among life’s greatest tragedies. Relationship reconciliation is too often left until there is no longer the ability for meaningful two-way communication. A legacy consultant will discuss the situation with both parties and attempt to mediate and restore a relationship. Not all relationships will be able to be repaired and restored. Relationships require a mutual desire to work towards understanding. However, it is possible to find peace and acceptance through reflection, forgiveness and attempted reconnection.
Personalized Future Communication
An enduring legacy is built of relationships and the meaningful moments and memories which comprise a well lived life. However, life’s journey does not always follow our plans. Sometimes there are things we want to say but, unfortunately, we will not be physically present to say them. A legacy consultant will help record the message and ensure it is delivered on that special day or life event. A video message on the day of a child’s wedding, a handwritten note on an 18th birthday, an audio recording sharing pride and love on graduation day, are a few of the ways to continue loving and inspiring those who mean the most to us when life’s journey is cut too short.
